Long Term Medicaid Managed Care

The NYS Medicaid homecare program changed from being run by New York State to privatization in 2012. Now private agencies are available to the consumer/Medicaid recipient to choose from. These agencies service the client's case and provide homecare services to the recipient. In the past the number of hours approved for care in the recipient's home was determined by the recipient's doctor, a home visit by the social worker with the CASA handling the case, and if more than 8 hours of care were to be assigned, a physician from Medicaid would also visit the home. This all changed when the program was privatized and now private Long Term Managed Care agencies determine the hours to be assigned.
Specifically, once a Medicaid application is approved, the recipient then contacts a Managed Long Term Care Agency. This agency will send an agency representative to conduct a home visit and will offer to provide a certain number of homecare hours per day or week for the recipient. If the hours are insufficient to meet the needs of the recipient, the recipient can contact another agency for another interview and another offer of hours of care. If the patient care needs exceed the capabilities of the agency, the agency can refuse to offer hours and services and direct the recipient to seek placement in a nursing home.
Since the inception of this program, I have noted that the recipients are receiving adequate hours of care to keep them safe in their own homes if desired. The program was put in place to reign in the out of control medical costs in New York State. There are some areas of the new system that are very helpful and beneficial to the recipients. The day care programs run by some of the Managed Care Agencies help to add socialization into the lives of otherwise shut in recipients. Many times the recipients enter the day program and locate long lost friends from their childhood and neighborhood, rekindling friendships. In the past, these people were cared for in their homes by one on one care with a home attendant. Adding the socialization of a day care center really does open up the lives of many recipients in a way that was absent from their lives for many years.
